A Personalized Care Plan
Your Case Worker does an in-depth evaluation to fully identify your needs, and then prepares an individualized Care plan to set goals for your health and other needs. Your Case Worker then works with you to track progress and to adjust the Plan as you progress
Community Resources
We can connect you with housing, employment, and education services and with medical care, mental health services, and rehab programs. Let us help you apply for Medicaid, disability benefits, and other financial programs
Advocacy and Support
We can advocate for you so that the system provides you with what you deserve. If you have a crisis or emergency, we provide immediate support and help to stabilize the situation. We offer guidance for family members and caregivers who are helping you.
Coordinated Services
Your Case Worker works closely with therapists, doctors, school officials, teachers, and social workers to make sure that your Care Plan works. Your Case Worker regularly reviews how the Care Plan is doing and can adjust the plan as needed. Your Case Worker makes sure that there are no gaps in care or communication.
Life Skills
Your Case Worker can show you ways to budget, to buy food and cook your meals, and to manage your time to make your daily life go better. Your Case Worker can help you get more education and job training, and generally help develop your career. Your Case Worker can create or show you opportunities to socialize and to be engaged in community activities to increase your sense of belonging
Tracking your progress in the Care Plan.
Your Case Worker regularly updates you (and your family) on the progress you are making in your Care Plan, carefully assessing the effectiveness of what you are doing and if the services you are receiving. Personal contact and follow-up are an important part of the Case Worker’s care.
